Either way, start addressing east from Limgrave to get to Caelid. Once you are in the new region, continue directly to the east and do not take the path that leads to the southeast or end in the Aeonia swamp.
You will spend the tumulus of dragons on your way to Fort Faroth and you can easily pass them through Torrent to get to the site of grace on the outskirts of the fort.

Once at Fort Faroth, your main goal is to reach the upper part of the tower to open a chest that contains half of the DECTUS medallion. The first floor is full of bats and two harpies that can destroy even at the highest levels, so we recommend that you run and take the stairs at the end of the room.
At the top of the staircase, open the chest to find the right half of the DECTUS Medallion. You can explore the rest of the fort to get more booty, or you can teleport you, since that is the main element you needed to get from this area.
If you have the other half of the Fort Haight medallion, you can activate the Great Decad Elevator to get to the Altus plateau.
